Alto riesgo de apnea obstructiva del sueño e insomnio clínico asociados con diabetes mellitus tipo 2: estudio de casos y controles
Resumen
Introducción: la diabetes mellitus tipo 2 es un problema de salud mundial. La evidencia reconoce al sueño como un componente de su manejo. Sin embargo, se desconoce si el riesgo de apnea obstructiva del sueño y el insomnio clínico se asocian con esta enfermedad en poblaciones latinoamericanas.
Objetivo: determinar la asociación entre el riesgo de apnea obstructiva del sueño y el insomnio clínico con la diabetes mellitus tipo 2 y evaluar la influencia de la obesidad en esta asociación.
Métodos: se realizó un estudio de casos y controles con 170 pacientes con diabetes mellitus tipo 2 y 170 controles apareados por edad y sexo. El riesgo de apnea obstructiva del sueño se evaluó mediante el cuestionario STOP-Bang y el insomnio clínico mediante el Índice de Severidad del Insomnio. Se realizaron regresión logística, apareamiento por puntaje de propensión y análisis de interacción entre apnea obstructiva del sueño y obesidad.
Resultados: el alto riesgo de apnea obstructiva del sueño se asoció independientemente con la diabetes mellitus tipo 2 (OR 5,71) y la asociación persistió tras el apareamiento por puntaje de propensión (OR 3,53). El insomnio clínico mostró una asociación independiente (OR 7,25), aunque el efecto fue más conservador tras la validación mediante bootstrap (OR 2,59). Se observó una interacción significativa entre alto riesgo de apnea obstructiva del sueño y obesidad (p 0,012), con una asociación más intensa en participantes sin obesidad (OR 9,14) que en aquellos con obesidad (OR 2,89).
Conclusión: el alto riesgo de apnea obstructiva del sueño y el insomnio clínico se asociaron independientemente con la diabetes mellitus tipo 2. La asociación entre alto riesgo de apnea obstructiva del sueño y diabetes fue significativamente más intensa en ausencia de obesidad, lo que respalda la existencia de un fenotipo metabólico relacionado con el sueño, independiente del paradigma centrado en la obesidad.
